The Mayo Clinic Health Assessment, a health risk appraisal, uses advanced online technology, providing a platform for personalization of each question set to the user, as well as providing individualized guidance for their unique set of risks and readiness.

Upon completion of the Mayo Clinic Health Assessment, individuals have access to My Health, a comprehensive and confidential action plan outlining their health status and health risks. My Health provides positive feedback and reinforcement on health strengths as well as direction for addressing employee health risks based on their readiness to change. My Health includes:

• An easy-to-understand Results Summary outlining the employee's health risk level (low, moderate or high)
• A comparison of individual risk results to recommended health scores, and a look at their health status compared to the last time they completed the Health Assessment
• A detailed report on each risk measured, including an explanation of the risk and actionable information on what can be done about it
• Links within My Health guide users to information and tools within the custom personalized health portal to address their specific employee health risks, and to organization-specific resources that address each risk identified
• Organization-specific links and health messages can be customized both within the text of My Health, as well as in a highly visible right-hand column that allows for visuals and longer text to accompany a link.
