The Mayo Clinic Health Assessment Physician Report is a useful tool in facilitating more meaningful and productive communication between a patient and doctor.
Mayo Clinic Health Assessment participants have access to a Physician Report to facilitate more meaningful, productive communication between patients and health care providers. The physician-friendly format of the report is designed to provide doctors with important, digestible pieces of information at a glance. The format is the result of the input from a team of experts led by Mayo Clinic Health Solutions’ medical director, Philip Hagen, M.D. Dr. Hagen and his team understood what pieces of information were most critical to include in this snapshot of a person's health according to their Health Assessment results.
With this information at the health care provider's fingertips, he or she will have a "wide angle" picture of the patient's overall health — instead of just an explanation of the reason for the visit (e.g., back pain).
The Health Assessment Physician Report is three to four pages long, depending on the participant’s survey responses. Contents of the Report include:
