The Mayo Clinic Health Assessment Physician Report is a useful tool in facilitating more meaningful and productive communication between a patient and doctor.
Mayo Clinic Health Assessment participants have access to a Physician Report to facilitate more meaningful, productive communication between patients and health care providers. The physician-friendly format of the report is designed to provide doctors with important, digestible pieces of information at a glance. The Health Assessment Physician Report is three to four pages long, depending on the participant’s survey responses. Contents of the Report include:
The Physician Report appears in the My Health section upon Health Assessment completion. The user simply clicks on the Physician Report pdf icon that can be saved and/or printed.
