The Mayo Clinic Health Assessment includes a comprehensive, confidential, report and action plan called My Health which provides users with details on health status, health risks, strengths and guidance to appropriate resources. Results for your organization are included in our EmbodyHealth Report Suite, a comprehensive menu of reports containing all of the information and you need to identify key population risks and follow through with appropriate health risk reduction and condition management programs. The EmbodyHealth Report Suite also lets you create customized reports for your entire population or by work site, and includes the following reports:

• Health Assessment Participation Report: Provides ongoing tracking of participation numbers for your Mayo Clinic Health Assessment campaign, including consent for incentive fulfillment and follow-up intervention.
• Health Assessment Group Report: A comprehensive summary of your population’s health status, health risks and readiness to change
• Health Assessment Benchmark Report: Compares your organization’s Health Assessment results to the Mayo Clinic Health Assessment database, allowing you to spot outliers in your population’s Health Assessment results
• Health Assessment Cohort Report: Provides trend tracking for Health Assessment participants who have two or more Health Assessment completions
