- Health Assessment Participation Report: Provides ongoing tracking of participation numbers for your Mayo Clinic Health Assessment campaign, including consent for incentive fulfillment and follow-up intervention.
- Health Assessment Group Report: A comprehensive summary of your population’s health status, health risks and readiness to change
- Health Assessment Benchmark Report: Compares your organization’s Health Assessment results to the Mayo Clinic Health Assessment database, allowing you to spot outliers in your population’s Health Assessment results
- Health Assessment Cohort Report: Provides trend tracking for Health Assessment participants who have two or more Health Assessment completions
- Health Assessment Intervention Report: An added enhancement to the standard report package that allows organizations to follow up with Health Assessment participants who consent to be contacted regarding health risks identified by the Health Assessment
